Enquire to be a new patient

Please email us at bnssg.168registrations@nhs.net to request a registration form and let us know your postal address so we can check you’re in our catchment area.

When we have received your registration request we will first check that you are within our catchment area before registering you as a patient (Please find the catchment area below). Please be aware that sending us a registration request does not automatically mean we are able to register you here. If you have any questions please don’t hesitate to get in contact with us via email at bnssg.168registrations@nhs.net or phone us on 01934 624242.

Please print out a registration form and new patient questionnaire (see link below), or alternatively, you can come into the surgery and fill out the forms in person.

Improved Access

We are working together with other local practices to offer patients access to more appointments in the early mornings, evenings and at weekends. To find out more about these services, please contact the practice.

Customer Notice

We are having a new phone installed on Friday 5th August. This should not affect your ability to contact the Surgery as the switch over should be seamless. Please bear with us if you experience any issues.

Surgery Closure

Due to a national problem with our clinical system, we are unable to open the surgery for the remainder of the afternoon (Friday 26th May).
