From Monday 3 August at 1:00pm, we will be introducing a new, revised format for Anima request forms for both patient and proxy submissions.
There will be no interruption to the Anima service, and the Anima dashboard will continue to work as normal. The only change is the format of the request forms.
The updated forms have been designed to:
Make it quicker and easier for patients to submit requests.
Help reduce processing and call handling time for our telephone hub, reception and patient coordinator team.
